Service to Consumers, Members or Outstanding State Chapter

Purpose

This award is given to the state chapter with documented activities in one of three categories: activities, publications, or service.

Criteria

  1. Submit a letter of nomination (no longer than 2 pages) describing the activities/publications/services provided and indicating, in specific terms, why the state chapter is outstanding.
  2. Include a copy of published materials pertaining to the item nominated.
  3. Specify on all materials the award for which the nomination is being made.
  4. Submitted chapter reports to AMHCA.
  5. Increased state and national membership.

Procedure

Any state president (or their designee) must submit materials documenting activities and a brief statement indicating why their state chapter should receive an award in the category.
